  1. Please fill out all New Patient forms in their entirety.  

  2. If you have any recent labs (within 12 months), please bring them to your appointment.   

  3. If you are married or in a relationship, please bring your spouse or significant other with you to your appointment. 

  4. There will be much information covered concerning your unique condition as well as the fundamentals of the program.  

  5. Please arrive on time. 

  6. We require a 24-hour notice to change or cancel your appointment.  

  7. Note: If these steps are not followed it may compromise the full value of your consultation and therefore we will kindly reschedule your appointment.

As soon as you make an appointment, our team will start planning to meet you either virtually or in office depending upon your convenience. We will take time to get to know you. The more we know about your health history, lifestyle, genetics, and symptoms, the more we understand what makes you, you.

We will obtain comprehensive lab work and specialty testing like microbiome, food sensitivity, and may be genetic testing to give us a deeper look at different aspects of your health, and uncover the cause of a persistent symptom.

Receive your tailored health plan, created by our team, which includes recommendations on nutrition, lifestyle adjustments, supplements, and more. We also focus on reassessing the current medications and lower the number of meds.

Advanced Testing 

Our team creates personalized wellness programs that include advanced testing and analysis. Advanced testing provides key diagnostic and treatment information to help reveal the cause of illness. We use a combination of traditional and functional testing to get the necessary information. Tests may use blood, saliva, urine, or stool. These may include:​
